Pre-Inspection Form Disclosure 

If you are a practitioner, APRN, clinic, or pharmacy that will purchase controlled substances for the purpose of dispensing or administering, you may subject to an inspection and/or policy review before your application or request may be processed.  This does not apply to those that will be administering or dispensing another healthcare registrant’s stock but rather their own. 

Please complete the Pre-Inspection Form and attach it with your application or Change of Information form.
